Dear Parent/Guardian,
We are excited about the upcoming 2023-2024 school year at Jefferson County K-12 School, and we would like to get a head start in preparation for the first day of school. Listed below are some of the health requirements for all students who attend schools in Florida.
Please be aware that all students, either current or new, will be required to have a current Department of Health School Entry Physical form and a current Department of Health Certification of Immunization Record form prior to the first day of school. These requirements are listed in Chapter 1003.22, Florida Statutes, and 64D-3.046, Florida Administrative Code. We will need your child’s School Entry Physical Form and Certification of Immunization Record Form before classes begin on August 10, 2023. If your student does not have these records completed and turned in prior to the first day of school, they will not be allowed to enroll for the new school year.
Please follow up with the school registrar to make sure that your child is current on their health records for school entry. These forms are available from your child’s pediatrician or primary care doctor. If you need assistance in finding a pediatrician or primary care doctor, please call the Jefferson County Health Department at (850) 342-0170.
To assist you with meeting these requirements, we are hosting a school physical and immunization fair on June 8thand 9th from 9 am until 1 pm at the Turning Point school facility at 575 South Water Street, Monticello, Florida 32344. School physicals will be provided by Florida State University, College of Medicine and limited immunizations will be provided by the Jefferson County Health Department. Please see the attached brochure for more information. If your child needs an immunization that is not offered during this event, please call (850) 342-0170 for an appointment.
