🪥🦷 The Jefferson County Health Department will be offering a 🆓 preventative dental program screening along with a 🆓 school based oral health program screening which includes silver diamine fluoride treatment and will also be administering 🆓 flu vaccines at the Jefferson K12 School Clinic on Friday, October 25, 2024. 💉
✍️ Please complete the four (4️⃣) forms below and return them to the school with your child no later than Wednesday, October 23, 2024. (These forms have also been e-mailed to your Focus Parent Portal Account & are available for download)
- Jefferson Dental Outreach Program Participation (5 pages)
- School-Based Oral Health Program Consent Form (1 page, available in both English & Spanish)
- Silver Diamine Fluoride Treatment Consent Form (1 page, available in both English & Spanish)
- Vaccine Consent Form (1 page, available in both English & Spanish)
