šŸŖ„šŸ¦· The Jefferson County Health Department will be offering a šŸ†“ preventative dental program screening along with a šŸ†“ school based oral health program screening which includes silver diamine fluoride treatment and will also be administering šŸ†“ flu vaccines at the Jefferson K12 School Clinic on Friday, October 25, 2024. šŸ’‰

āœļø Please complete the four (4ļøāƒ£) forms below and return them to the school with your child no later than Wednesday, October 23, 2024. (These forms have also been e-mailed to your Focus Parent Portal Account & are available for download)

- School-Based Oral Health Program Consent Form (1 page, available in both English & Spanish)
